Health Office Notes
Connecticut state law requires a physical examination for all students during the sixth grade.  A completed “blue form” must be submitted in order for the child to enter grade seven.  The physical should be dated no earlier than January 1, 2010.
The state law also requires that the sections of the chronic disease assessment, vision, hearing, and postural screenings be completed by the health care provider.  Incomplete forms will be returned to parents.  Blue forms are usually provided in the Doctor’s office, but are also available in the Health Office.

The following are immunization requirements for entry into 7th grade:

*Three doses of Hepatitis B
*A second MMR (measles, mumps, and rubella)
*One varicella  (2nd varicella after age 13) or verification of disease by MD
